The Council Rock Coalition for Healthy Youth (CRCHY) is working to promote a healthy environment for children. The following survey will provide it with information to better serve the community. The survey is anonymous. Participation is crucial to ensure accurate results so it can plan programs and activities that meet the needs of the community. CRCHY asks residents to forward the link to friends and neighbors and encourage them to participate.
The beginning questions, numbers 1–5, are for all who work or live in the Council Rock community. Parents of children in the Council Rock School District should continue and answer the remaining questions.
